PURPOSE: To evaluate 12 month surgical outcome of Kahook Dual Blade (KDB) goniotomy in combination with cataract surgery in Latino patients with open angle glaucoma (OAG) and ocular hypertension (OHT). METHODS: This retrospective study included 45 eyes of 40 patients who underwent KDB goniotomy combined with cataract extraction from January 2016 to September 2020 at two centers in South America. Primary outcome was surgical success defined as ≥ 20% intraocular pressure (IOP) reduction or ≥ 1 medication reduction from preoperative without additional IOP-lowering procedures and an IOP ≥ 5 mmHg or ≤ 21 mmHg. Additionally, we used 2 cutoffs values for success of IOP ≤ 18 and ≤ 15 mmHg. Secondary outcomes included: IOP, medication use, best corrected visual acuity, complications and failure-associated factors. RESULTS: Success rates at 12 months with cutoff limits of 21, 18 and 15 mmHg were 84.3%, 75.6% and 58.7%, respectively. At 12 months, mean preoperative IOP significantly decreased from 19.23 ± 0.65 mmHg on 2.3 ± 1.0 medications to 14.33 ± 0.66 mmHg on 0.6 ± 0.9 medications (p < 0.001) , with 62% of eyes free of hypotensive medication. Eyes that developed postoperative IOP spikes showed a higher risk for failure using the cutoff limit of IOP ≤ 18 mmHg with a hazard ratio of 3.6 (95% confidence interval [CI], 1.80-7.13; p < 0.001). There were no serious ocular adverse events. CONCLUSIONS: KDB combined with cataract extraction showed safety and efficacy for decreasing IOP in OAG and OHT Latino patients. Additionally, dependence on medications was reduced significantly after surgery.

PURPOSE: This study evaluated the agreement and reproducibility of ACA measurements obtained using the built-in software of the CASIA2 (Version 3G.1) and the measurements derived from expert clinicians. METHODS: Healthy volunteers underwent ophthalmological evaluation and AS-OCT examination. ACA measurements derived from automated and manual SS location were obtained using the CASIA2 automated software and clinician identification, respectively. The intraobserver, interobserver reproducibility, CASIA2-human grader reproducibility and CASIA2 repeatability were assessed using intraclass correlation coefficients (ICCs). RESULTS: The study examined 58 eyes of 30 participants. The CASIA2 software showed excellent repeatability for all ACA parameters (ICC > 0.84). Intraobserver, interobserver, and CASIA2-human grader reproducibility were also excellent (ICC > 0.87). Interobserver agreement was high, except for nasal TISA500, differing between observers 1 and 2 (p < 0.05). The agreement between CASIA2 measurements and human graders was high, except for nasal TISA500, where observer 1 values were smaller (p < 0.05). CONCLUSION: The CASIA2 built-in software reliably measures ACA parameters in healthy individuals, demonstrating high consistency. Although a small difference was observed in nasal TISA500 measurements, interobserver and CASIA2-human grader reproducibility remained excellent. Automated SS detection has the potential to facilitate evaluation and monitoring of primary angle closure disease.

Aim and background: Precision of optical coherence tomography (OCT) measurements of the optic nerve head (ONH), retinal nerve fiber layer (RNFL), and macular ganglion cell layer (GCL) is essential for the diagnosis and monitoring of glaucoma. The purpose of this research was to evaluate the repeatability and reproducibility of retinal and ONH parameters measured with two identical swept-source optical coherence devices. Methods: A cross-sectional study was conducted. A total of 30 eyes of 15 healthy subjects were included. Two technicians performed four OCT-wide protocol scans in the same visit using two identical Triton swept-source OCT (DRI-OCT) instruments. The interdevice and interobserver reproducibility and the repeatability of both instruments for all ONH, RNFL, and macular GCL parameters were evaluated by the intraclass correlation coefficient (ICC). Additionally, Bland-Altman test analysis was used for repeatability and reproducibility measurements. Results: Intraclass correlation coefficient (ICCs) of the ONH, RNFL, and GCL measurements were excellent for repeatability and interdevice reproducibility (>0.9). Interobserver reproducibility was good for all parameters except for RNFL clock hour 11 (ICC = 0.72). The variability of the average RNFL was from -4.103 to 4.97 µm, with a mean percentage of the difference (PD) of 0.37 ± 2.03%. Among GCL parameters, the greatest variability was found in the inferior sector (PD = -0.88 ± 5.39%, limits of agreement (LoA) = -8.345-7.078 µm). Conclusion: Using two identical swept-source OCT instruments for the evaluation of the structural parameters of the ONH, RNFL, and macular GCL showed high repeatability and reproducibility. This allows the clinician to make a therapeutic decision based on OCT findings coupled with the clinical evaluation of the patient. When evaluating RNFL clock hours measurements, interobserver reproducibility might decrease. Clinical significance: The understanding of measurement variability while using different devices and the impact of the observer capturing the images, is clinically relevant. AIM: To describe the safety and efficacy of patterned laser trabeculoplasty (PLT) as an adjunctive treatment in primary open angle glaucoma (POAG) and ocular hypertension (OHT) after 18-month follow-up in Hispanic population. METHODS: A single-center, retrospective study was conducted. All patients with OHT or POAG undergoing PLT from June 2016 to August 2016 were included in the study. Investigated parameters were intraocular pressure (IOP), the number of IOP-lowering medications, best corrected visual acuity (BCVA), laser parameters and postoperative adverse events. Primary efficacy outcome measures were the proportion of eyes achieving an IOP reduction ≥20% at 18mo versus baseline medicated IOP or a reduction in the number of medications while maintaining IOP values. RESULTS: From 40 PLT-treated eyes (mean baseline IOP 20.3±1.7 mm Hg), 24 patients were analyzed (age 63.4±7.3y). The mean IOP reductions from baseline across visits (months 1, 3, 6, 9, 12, and 18) ranged from 14.1% to 20.8%. Success rate after 18-month follow-up was 61.7% with a mean IOP of 16±3.2 mm Hg (P<0.001). The number of glaucoma IOP-lowering medications per eye (preoperative 2.1±1.1 and postoperative 2.3±1.1, P=0.86) and the mean BCVA (preoperative 0.10±0.22 and postoperative 0.11±0.22 logMAR, P=0.42) remained stable. Adverse events comprised transitory IOP spikes in 4 eyes (10%) and peripheral anterior synechiae in 7 eyes (17.5%). CONCLUSION: Mid-term results of PLT show that this procedure may be an efficacious and safe technique to approach medically uncontrolled OHT or POAG patients.

AIM AND OBJECTIVE: To present a case of bilateral delayed-onset hyphema following the administration of a 1% tropicamide and 2.5% phenylephrine fixed combination ophthalmic agent, in the late follow-up period of a gonioscopy-assisted transluminal trabeculotomy (GATT) combined with cataract extraction. BACKGROUND: Gonioscopy-assisted transluminal trabeculotomy consists on a 360° trabeculotomy through an ab interno approach that may also be combined with cataract surgery. Delayed-onset hyphema has been reported with trabecular minimally invasive glaucoma surgery (MIGS) procedures. Some proposed mechanisms are ocular compression and decompression during sleeping on the surgical side and episcleral venous pressure rise after physical activity. CASE DESCRIPTION: We describe the case of a 68-year-old female patient with ocular hypertension (OHT) and bilateral cataracts who underwent uncomplicated combined GATT and cataract extraction surgery. Postoperatively, 8 months after the left eye (OS) surgery and 3 months after the right eye (OD) surgery, patient came for routine evaluation. After induced mydriasis, slit-lamp evaluation revealed the presence of 3+ OD and 4+ OS erythrocytes in the anterior chamber (AC). Prednisolone acetate was prescribed q.i.d. and remission of hyphema was achieved after 2 weeks. Subsequently, 4 months later, the pupil dilation was again induced showing 4+ erythrocytes in both eyes (OU), layered hyphema in the inferior quadrant OS, and intraocular pressure (IOP) spike OU. The intraocular pressure was controlled after oral acetazolamide was prescribed. Topic prednisolone was initiated, and after 1 week, the hyphema was resolved in OU. CONCLUSION: Delayed-onset microhyphema may occur following induced mydriasis even months after the uncomplicated GATT procedure. Ophthalmologists should be aware of the possibility of microhyphema after induced mydriasis and the risks that this might represent with noteworthy and repeated IOP spikes which may eventually require treatment. CLINICAL SIGNIFICANCE: Delayed-onset hyphema and IOP spikes may occur following the pupil dilation with fixed combination of phenylephrine and tropicamide ophthalmic agent after the uncomplicated GATT procedure. Actualmente hay descritos 2 métodos para diagnosticar Demodex. Uno de ellos la depilación y visualización con microscopio de luz y el otro sin depilación, rotando las pestañas. Este caso plantea una nueva alternativa diagnostica para visualizar los ácaros.


Currently there are 2 methods described to diagnose Demodex. One is depilation and visualization with light microscopy and the other is without depilation by rotating the eyelashes. This case presents a new diagnostic alternative to visualize the mites.

AIM: To assess the efficacy and safety of patterned laser trabeculoplasty (PLT) as an adjunctive treatment in open angle glaucoma (OAG) or ocular hypertension (OHT) patients who were under antiglaucoma medical treatment. METHODS: This study was a retrospective review of primary or secondary OAG patients and OHT patients with medically uncontrolled (≥18 mm Hg) intraocular pressure (IOP) who underwent 360° PLT from June 2016 to August 2016. Follow-up visits at week 1, and 1, 3 and 6mo were performed. IOP, best corrected visual acuity (BCVA), complications and eye drop glaucoma medication were recorded at each follow-up visit. Success was defined as IOP reduction ≥20% from baseline. RESULTS: Forty-one eyes of 25 patients were included in this study. Pre-treatment mean IOP was 20.2±1.6 mm Hg. After PLT, IOP was 19.3±5.2, 16.1±2.7, 17.1±3.7 and 16.3±3.5 mm Hg, at 1wk, 1, 3 and 6mo, respectively. IOP reduction from baseline was statistically significant from the first month, remaining stable at 6mo (P<0.001). PLT success at 6mo of follow-up was 48.78%. The number of glaucoma medication per eye (P=0.10) and the mean BCVA both remained constant (P=0.37). Complications included transient IOP spikes in 4 eyes (9.8%) and peripheral anterior synechiae in 7 eyes (17.1%). CONCLUSION: PLT is an effective and safe method for the management of patients with OHT or OAG as an adjunctive therapy. Additional larger studies should be designed to verify the long-term stability of IOP reduction with this laser technology.

Introduction: The mortality rate in low rectal cancer is related to pelvic and distant recurrence. For stage I tumors, local excision has being used increasingly, but recent studies show the need for caution with the use of this technique, as they do not consider the possibility of a positive node in stage I rectal tumors. Therefore, preoperative radiotherapy should be considered for early tumors, as an attempt to prevent recurrence. Objetive: Show the effectiveness of neoadjuvant radiotherapy in stage I cancer of the lower rectum of a cohort population. Material and method: A cohort study in a prospective database was made with a total of 538 patients, of which were considered 75 patients with stage I lower rectal cancer. Preoperative radiotherapy was performed and patients were followed up for a minimum period of five years. Results: Stage I/TI group had 27 patients. All of them presented complete response to the treatment and did not need to be operated. During the follow up time of five years, this group showed no recurrence rate. The stage I/TII group had 48 patients. During the follow up, 8 patients had to be operated due to suspicious lesion or scar. They were submitted to full total local excision. After evaluating the pathological specimen, none of them proved to be adenocarcinoma. Conclusion: Preoperative radiation, not only reduced the local recurrence and mortality rate in lower rectal cancer, but also reduced the need for surgery in patients with stage I cancer.


Introdução: O percentual de mortalidade em pacientes com câncer de reto baixo está relacionado a recorrências pélvica e remota. No caso de tumores no estágio I, a excisão local vem sendo utilizada cada vez mais; contudo, estudos recentemente publicados demonstraram a necessidade de se ter cautela com o uso dessa técnica, por não se levar em consideração a possibilidade de um nodo positivo em tumores de reto no estágio I. Portanto, a radioterapia pré-operatória é uma opção viável para os tumores em fase inicial, como uma tentativa de evitar recorrência. Objetivo: Demonstrar a eficácia da radioterapia neoadjuvante em casos de câncer de reto baixo no estágio I em uma coorte da população. Materiais e métodos: Foi realizado um estudo de coorte em um banco de dados prospectivo, com envolvimento, no total, de 538 pacientes, dos quais 75 foram considerados como tendo câncer de reto baixo no estágio I. No pré-operatório, os pacientes foram tratados com radioterapia e seguidos durante um período mínimo de 5 anos. Resultados: O Grupo no estágio I/TI consistia em 27 pacientes. Todos obtiveram resposta completa ao tratamento e não houve necessidade de reoperação. Durante o período de 5 anos de seguimento, não houve recorrências nesse grupo. O grupo no estágio I/TII consistia em 48 pacientes. Durante o seguimento, 8 pacientes tiveram que ser operados, devido à suspeita de lesão, ou cicatriz. Para esses casos, optou-se por excisão local total completa. Após a avaliação dos espécimes patológicos, nenhum deles teve diagnóstico de adenocarcinoma. Conclusão: O uso da radiação pré-operatória não só diminuiu a recorrência local e o percentual de mortalidade em casos de câncer de reto baixo, mas também diminuiu a necessidade de cirurgia em pacientes com câncer no estágio I.

La silicosis es una enfermedad sin tratamiento. El programa de vigilancia de trabajadores expuestos a sílice está diseñado para detectarla lo más precozmente posible mediante una radiografía de tórax. A pesar de su rol fundamental, esta adolece de una gran variabilidad técnica y de interpretación. Para disminuir estos fenómenos, la OIT creó un sistema de clasificación ampliamente usado en Chile y el mundo. Este se basó en la radiografía análoga, la que ha sido reemplazada por el mismo set análogo, digitalizado. Si bien la OIT permite el uso de este nuevo tipo de imagen, indica que el patrón de comparación debe seguir siendo el análogo. Muchos estudios han demostrado que es mejor utilizar patrones de comparación digitales. El objetivo de este trabajo fue generar un set de radiografías digitales que representen las profusiones centrales de opacidades redondas del sistema de clasificación OIT. De un universo de 32.826 radiografías de pacientes expuestos a sílice, se obtuvo 252 radiografías, de las cuales 54 fueron informadas por 3 lectores acreditados, seleccionándose 8 casos que contaban con opacidades redondas puras de diferentes profusiones, las que se proponen como patrón digital para Chile.


Silicosis is a disease without treatment. The surveillance program for workers exposed to silica is designed to detect as early as possible by a chest radiograph. Despite its fundamental role, it suffers from a great technique and interpretation variability. To reduce these phenomena, the ILO created a classification system widely used in Chile and the world. This was based on analog radiography, which has been replaced by the same analog set, digitized. While the ILO permits the use of this new type of image it indicates that the pattern comparison must remain analog. Many studies have shown that it is best to use digital comparison patterns. The objective of this work was to generate a set of digital x-rays representing the central profusion of round opacities of ILO classification system. From a universe of 32,826 x-rays of patients exposed to silica, 252 films, of which 54 were reported for 3 accredited readers, selected 8 cases that had pure round opacities of different profusions was obtained, which are proposed as digital pattern for Chile.

In recent decades, many compounds with central dopaminergic activity have been designed, synthesized and evaluated pharmacologically. However, it has not been possible to obtain a drug able to improve or cure diseases involving dopaminergic regulation in the central nervous system, such as Parkinson's disease and schizophrenia, among others. Taking into consideration the term "atypical pharmacophore" and from the compound 5, the aralkyl fragment was incorporated, and the compounds 10, 11, 13a-h and 14a-h were synthesized. Both the compounds 10 and 13a-h under its methoxylated form and the compounds 11 and 14a-h under the phenolic form, were evaluated to determine their pharmacologically agonistic and antagonistic effects on central dopaminergic activity. For this, the effect of intracerebroventricular injection of said compounds on the hydromineral balance and stereotyped behavior in rats, was determined. The results of the preliminary pharmacological evaluation show a centrally acting action through dopamine mechanisms, in which the compounds 10, 11, 13d-h and 14a showed responses as agonists, whereas compounds 14b-h, had responses as antagonists.

En las últimas décadas son muchos los compuestos con actividad dopaminérgica central que se han diseñado, sintetizado y evaluado farmacológicamente. A pesar de ello, no se ha logrado obtener un fármaco capaz de mejorar o curar las patologías que involucran la regulación dopaminérgica en el sistema nervioso central tales como la Enfermedad de Parkinson y la esquizofrenia, entre otras. Tomando en consideración el término de “farmacóforo atípico” y a partir del compuesto 5, se incorporó el fragmento aralquil y se sintetizaron los compuestos 10, 11, 13a-h y 14a-h. Tanto los compuestos 10 y 13a-h bajo su forma metoxilada como los compuestos 11 y 14a-h bajo su forma fenólica, fueron evaluados farmacológicamente para determinar su actividad agonística y antagonística sobre el sistema dopaminérgico central. Para ello se determinó el efecto de la inyección intracerebroventricular de dichos compuestos sobre el balance hidromineral y la conducta estereotipada en ratas. Los resultados de la evaluación farmacológica preliminar muestran una acción central a través de mecanismos dopaminérgicos, siendo que los compuestos 10, 11, 13d-h y 14a mostraron respuestas como agonistas, mientras que los compuestos 14b-h, tuvieron respuestas como antagonistas.

El presente trabajo fue realizado en los trabajadores de servicio de la UniversidadNacional Autónoma de Honduras (Ciudad Universitaria), con el objetivo de conocerla prevalencia de Hipertensión Arterial en dicho personal, se realizó en el periodocomprendido de Enero a Junio del 2012; es un estudio descriptivo de cortetransversal, se contó con un universo de 634 trabajadores y una muestra de 192,se utilizó un instrumento (Cuestionario) el cual se aplicó previo consentimientoinformado; obteniendo los siguientes resultados: el 18% de las personasentrevistadas su edad es de más de 50 años seguida de un 17% entre 30 y 44años, un 15% entre 25 y 29 años, un 9% entre 19 a 39 años.(Tabla No.1) En elestudio participaron un 62.5% del sexo femenino 37.5% del sexo masculino; el100% son de la raza mestiza un 55.2 % son evangelicos,un 37.5% son católicos,un 5.20% no profesa ninguna religión; la mayoría su estado civil es soltero, seguidode los casados y unión libre; la procedencia de los trabajadores en un 83.33% esurbana y un 16.66% es rural; en cuanto a nivel educativo la mayoría solo tienen elnivel primario seguido del nivel secundario y en una minoría nivel universitario; encuanto al ingreso mensual la mayoría de ellos devengan de 7,000 a 8,000Lempiras. En referencia al nivel de conocimiento sobre HTA se ve que tienen conocimientossobre los factores de riesgo, no así sobre los síntomas de la enfermedad, de loscuales los que más conocen son: dolor de cabeza, mareos y visión borrosa.
